    Medium-chain specific acyl-CoA dehydrogenase (MCAD), encoded by the ACADM gene, catalyzes the initial step in mitochondrial fatty acid beta-oxidation—a critical energy-producing pathway that breaks down fats into acetyl-CoA. Specifically, MCAD removes hydrogen atoms from C-2 and C-3 of saturated medium-chain acyl-CoAs (6–12 carbons), generating trans-2-enoyl-CoA while transferring electrons to the respiratory chain via electron transfer flavoprotein (ETF). Located in the mitochondrial matrix, MCAD enables aerobic energy derivation from lipids, and its dysfunction is linked to metabolic disorders.
