    Plecanatide is a synthetic analog of the endogenous peptide uroguanylin, differing by a single amino acid substitution where aspartic acid at position 3 is replaced with glutamic acid. It functions as a guanylate cyclase-C (GC-C) receptor agonist, stimulating the production of cyclic guanosine monophosphate (cGMP) in a pH-dependent manner. Structurally, plecanatide maintains the disulfide bond-forming cysteine residues characteristic of uroguanylin, which are critical for its biological activity.

    The peptide exhibits potent cGMP-stimulatory activity in human colon carcinoma T84 cells, with an EC50 of 1.9 × 10-7 mol/L. Its mechanism involves activation of GC-C receptors localized on the apical surface of intestinal epithelial cells, influencing ion transport, mucosal homeostasis, and barrier function. Plecanatide demonstrates resistance to proteolytic degradation, enabling its functional stability in the gastrointestinal lumen.
