    Big LEN is a peptide derived from the C-terminal region of the proprotein convertase subtilisin/kexin type 1 inhibitor, commonly known as proSAAS or PCSK1N. This peptide corresponds to the sequence LETPAPQVPARRLLPP, though the N-terminal leucine is frequently absent in mass spectrometry analyses of cerebrospinal fluid, resulting in the commonly detected fragment ETPAPQVPARRLLPP. ProSAAS is a neuronally and endocrinely expressed protein that functions as a chaperone, inhibiting the aggregation of amyloidogenic peptides.

    In the context of Alzheimer's disease, cerebrospinal fluid levels of proSAAS and its derived Big LEN peptide are consistently decreased. This reduction is observed across multiple independent proteomic studies and is supported by statistical significance. The consistent downregulation of this specific peptide fragment suggests it may reflect altered neuronal processing or retention of the parent protein within pathological brain aggregates, rather than being an artifact of sample preparation.
